Indications

Pradaxa is an anticoagulant medication used to prevent blood clots, treat deep vein thrombosis (DVT) or pulmonary embolism (PE), and lower the risk of stroke caused by blood clots in those with atrial fibrillation not caused by a heart valve problem.

Dabigatran etexilate is a direct thrombin inhibitor that inhibits platelet aggregation and vasodilation by increasing cyclic adenosine monophosphate levels through its inhibitory effects. This allows Pradaxa to offer antithrombotic effects while improving blood flow to prevent blood from clotting.

This anticoagulant is generally well-tolerated and effective as a preventative measure for stroke and systemic embolism in those with nonvalvular atrial fibrillation. Pradaxa is also associated with providing lower rates of serious adverse reactions.

Uses and Dosage

The dose of Pradaxa that is prescribed will vary depending on the individual being treated. Follow all your doctor’s instructions and read packaging labels carefully to ensure the best results during your treatment.

  • To prevent DVT and PE after hip replacement surgery, most take 110mg for the first day and then 220mg once daily

  • Do not suddenly stop taking Pradaxa
  • Store at room temperature away from moisture and heat

Cautions

If you undergo a spinal tap or receive spinal anesthesia while using Pradaxa, a very serious spinal blood clot could form. The risk of this is higher if you have a genetic spinal defect, a spinal catheter, a history of spinal surgery or repeated spinal taps, or if you also use other drugs that affect blood clotting.

Do not suddenly stop taking Pradaxa because your risk of blood clotting or having a stroke will be increased. Consult with your doctor if you want to stop taking this medication.

Do not use Pradaxa if you have an artificial heart valve or active bleeding.

Dabigatran etexilate may cause you to bleed more easily, especially if you have any of the following:

  • Stomach ulcer or bleeding in your stomach or intestines
  • Kidney disease (especially if you also take dronedarone or ketoconazole)
  • If you take medicines that increase your risk for bleeding such as aspirin, clopidogrel, or heparin
  • If you take a nonsteroidal anti-inflammatory drug (NSAID) regularly such as ibuprofen, naproxen, or diclofenac
  • You are older than 75

To ensure your safety when taking Pradaxa, tell your doctor if you have any of the following:

  • Kidney disease
  • Bleeding disorder caused by disease or genetics
  • Antiphospholipid syndrome

Tell your doctor if you also take rifampin, amiodarone, clarithromycin, dronedarone, or ketoconazole before taking Pradaxa.

Some other drugs can interact with dabigatran etexilate and cause adverse reactions. To ensure your safety, tell your doctor about all the medications you take, including over-the-counter medicine, vitamins, and herbal products, but especially any of the following:

  • Abciximab, clopidogrel, dipyridamole, eptifibatide, ticlopidine, tirofiban
  • Alteplase, reteplase, tenecteplase, urokinase
  • Apixaban, argatroban, bivalirudin, desirudin, lepirudin, rivaroxaban
  • Dalteparin, enoxaparin, heparin, tinzaparin

Pradaxa may cause bleeding in the mother or baby when taken during pregnancy, but the risk of developing blood clots is higher during pregnancy and is important to prevent. Tell your doctor if you are pregnant or plan to become pregnant to determine a treatment plan that’s right for you.
